Accessing the Services

• Medical consultation (at the scheduled time);
• Communicating the schedule for the entire treatment period;

1. Copy of the child's birth certificate; (only when accessing the services for the first time)
2. Copy of the parents' identity documents; (only when accessing the services for the first time)
3. Copy after the divorce sentence (if applicable);
4. Copy of the child's medical documents (exit tickets - from which the medical history can be obtained); (starting with the second access to the services, only the child's most recent medical documents are brought)
5. Original certificate from the family doctor stating that "the child can be integrated into the community";
6. Medical analyzes (coproparasitological and pharyngeal exudate); (valid for 6 months)
7. Copy of the valid medical certificate, from the specialist doctor;
8. Copy of the valid decision of the Commission (Complex Evaluation Service (SEC) within the General Directorate of Social Assistance and Child Protection (DGASPC) of domicile);
9. Copy of the valid certificate of placement in the degree;
10. Copy of the valid recovery plan;
11. The contract with the family issued by the DGASPC in the area of residence (contract, if any, between the case manager and the child's legal representative).

(Acts 8,9,10) only for children who have a file at the DGASPC (General Directorate of Social Assistance and Child Protection) in the area of residence.
Any of the documents mentioned above, which change or lose their validity, must be updated.

• towel and robe for hydrotherapy, shower gel;
• oil and large towel for massage;
